Membership Certification and Fellowship Elevation

Regulations pertaining to Membership Certification and Fellowship Elevation can be found in the Training Program Handbook

To apply for Membership or Fellowship, please contact the Training Services Officer at College House, who will prepare an application form for you.  This is because the application form has to list all your rotations and the exact amount of time you spent at each.  There is no application form to be downloaded from the website.
